NEW CLIENT APPLICATION (Play Workshop) 

For your convenience we have provided the application packet for you to download in two different formats. If you have technical difficulties opening the application please let us know.

OPTION 1: PDF Package

This package contains each of the forms as a separate document allowing you to easily navigate between them. You can print them indivdually or all at once. You will need up-to-date Adobe Reader and Flash to view it properly. If you have a Mac you may not be able to view the files in this format.

OPTION 2: PDF single document

Here the documents are combined into one large pdf file. You will need to scroll down to find each document. You should choose this option if you are having difficulty with viewing the pdf package.

TIPS FOR COMPLETING THE FORMS

Many of the forms are "fill-able," meaning you can complete them on your computer. However, unless you have a pdf writer program installed you will not be able to save the text that you enter. Please PRINT a copy of each form as you complete it.

RETURNING THE APPLICATION PACKAGE

You have 3 options for returning the application package. We do not recommend returning the application via email as it is not a secure way to send personal information.
